ha ha, umm... well...
sometimes they are totally random, & thats the worst, you never know when its going to happen & its very embarissing, you could be anywhere thinking about anything, & then it just sort of happens...
sometimes when we're thinking about sex or see something we like, then that will do it too
or sometimes, something will hit us somewhere or what ever & it feels nice, & that will also do it

I speak as a 21 year old male who rarely becomes aroused by thoughts of sex or sexual contact but just a passionate kiss from my lover will set me on fire. Some poeple are excited by different things. Some guys like breast, some like butts, some like feet. Depends on a few things: his age, his libido, and how long its been since he last ejaculated.
Until the end of the teenage years it is utterly random. Absolutely nothing can cause them, and it can happen often too.
The older you get, the more recently you climaxed etc. the more it takes to get one. By this, I mean the more sexual stimulation (mental or physical) you require.
I personally disagree that guys think about sex most of the time. Personally I think women made that one up. Guys might check girls out or whatever, but the conscious thought of sex is not a neccessity.
Anyway, to conclude, if its a teen then likely he could be thinking about the latest tax reforms suggested by congress. If its an adult, its probably (not always) some form of sexual thought.

Some times when boys are young they get boners at random times. Like while taking a test or at their grandmas house. hehe Its just part of puberty. But when they get older, the get it when they see sometihng that turns them on.

Do they think of much else? Sex is it. They think of it a lot. Anything else they may be thinking doesn't cause it...well, unless they are some sort of weirdo. :)
