If you are sent an invitation to someone's Sweet 16 and the card says RSVP, do you have to call to inform them that you will not be attending?

No, you don't have to call, but you do have to respond. Another way of responding is send them a note or at least email them. When someone puts RSVP on an invitation they expect some kind of response. You can say something like; I regretfully decline the pleasure of attending your party. Best wishes to you on your 16th birthday and may your day be a special one. And then, your name. You don't have to be elaborate or render any explanation as to what you will be doing that day. It's none of their business. You can also say, unfortunately I will not be able to attend. Best wishes to you on your special day. Clean and simple. Responding in some way will show you have character and a person who is considerate of others. This also helps this person's family plan food, beverages, etc. for the party when they have definite responses. Learning to do these things now will help you when you get out into the business world. This is an important practice in all matters of business as well as life and if you start practicing it now you will go far. I know this sounds strange but people judge what kind of person you are by how you respond to these simple gestures. When it truly gets down to it, no, you don't HAVE to do anything about it, but the simple gesture will accomplish a lot in terms of the opinions of others and you never know when you may need this families high opinion. Good luck!

Namaste!

it is not required, but it is always better that you do so that they do not wait!(p.s. its a polite thing to do)

Most of the time, yes.
Or you could tell them in person, but it probably means call so they can add you to the list.
