Yeah I've written a bunch. First you should start off by thanking them - you know, let them know that their work, time, and effort is appreciate. Thank them for a great school year and for being your teacher. You can also thank them for putting up with you. At this point, or maybe a little more on, you can apologize simply by saying something like: I would like to apologize to you for my misbehavior (or whatever it is you want to apologize for). State that you know what you did was wrong and that you will do your best to try not to do it again. Remember to be formal, because this is not a letter to your friend, and you want to be sincere - let them know you mean it. Oh and of course, wish them a happy summer as a closing, hoping to see them again next year. I think with that on the letter and anything else you feel is necessary to throw in (properly placed of course), should do it. Hope that helped and good luck.
