I think it goes beyond a simple yes or no answer though. You know they're obviously not okay and you asking them if they are is a sign they can open up to you and tell you what's wrong.
I don't know if everyone has the same thoughts on this as me though!
People crying is quite an awkward situation for some.
If you're comfortable with it, console them. If they open up to you, listen and offer a helping hand, a hug, and tell them everything will be okay. but it also depends on why they are crying and who the person is.
If it's an acquaintance, ask them what's wrong. If they tell you, they want your help. If they show no sign of wanting you to know, don't interrogate and make them tell you. Just tell them things will work out and perhaps stay with them until they stop.
If it's a stranger, asking what's wrong would be a little rude considering you don't know the person. This where I'd probably say the silly "are you okay" question. At least it shows you care. [ thelaura's advice column | Ask thelaura A Question ]

You could probably ask an aquantince/friend if they want to talk about it, etc.
For a stranger (or friend/aquantince) ask if there is anything you can do or someone that you can get for them.
Seriously though, I love this questions. It's kind of like one of those "What do you say when someone knocks on the bathroom door while you're in there..." haha. [ S_C's advice column | Ask S_C A Question ]
