how to write the permission notes

ok, so I can't pick up my report card from the school because I have to be away and the vice principal said it's ok for a friend to pick up my report card if I give him the permission note. But I don't know what I should write on the note, does it have to be professional?

Dear Sir or Ma'am;

I am unable to attend school on _________(date) and therefore unable to collect my report card. I spoke with Mr./Mrs.________________, who said that with this note, _____________________(my friend) is allowed to pick up my report card for me.

My signature verifies that I have made the necessary arrangements. Please allow ___________to receive my report on my behalf. Thank you.

Something simple like:

Dear [Vice Principal's name here]:

I, [insert your name here], do hereby allow permission for [insert friend's name here] to be able to pick up my report card from you.
If you have any questions, [Vice Principal's name here], please feel free to contact me at [insert cell phone, home phone, etc. here].

All you have to write is somting like
I (ur name here) give my permission for ( friends name) to pick up my report card, and then sign and date it

Hey, well i think that it should be professional. I only say this because i think that he wont take it seriously if its not. You should write it and then sign it. Also have your mom sign it.
