Birth Control I have to go on birth control because I keep getting headaches and the doctor said that it will put my hormones back in balance. My question is - Will they ask me if I am sexually active and if so, do I have to tell them. When I do tell them do they tell my mom?

Yes, they will most likely ask you if you are sexually active. You should answer truthfully because it may be of harm to you if you don't. You can request for them not to tell your mother. Because, what goes on in the room they check you out in is strictly confidential. Whatever you say cannot be repeated to your mother by LAW. ]
You should answer truthfully, but if you're over the age of thirteen (which I'm assuming you are), you have privacy rights and they cannot legally tell your mom that you are sexually active without your consent. They might ask, they might not. If you have any other questions, be sure to write them down and take them with you so you don't forget to ask them.

They will probably ask. It is good to tall your doctor the truth about things. They will not tell your mom about it.
Make a list of any questions you may have. You will have to get a pelvic exam (if you haven't already) before they will put you on the pill.
Its a great time to ask anything you are curious about. Write the questions down because you will be nervous (normal)and you don't want to forget anything.
