So me and my girlfriend were having sex, she has been on the Depo Proverba shot for about 4 months and has never been late. During one of our positions I realized that my condom had came off so we stop and I get it out. It was deep in so there was no way I could see what was in it, but I know that I never got off inside of her. Also I had urinated since the last time I got off so I feel like the pre-cum should be clean as well. After I got the condom out we completely stopped having sex, so should I be concerned about her being pregnant?

Having been on birth control for as long as she has should mean she is well protected against pregnancy. POrotectged to whatever the percentage is for the Depo shot is, which I believe is around 99%, the same as the pill.
I would not be too concerned at this time. In the future when wearing a condom just make sure it is on securely before having intercourse. The condom is just another layer of protection against pregnancy making an unwanted pregnancy as close to 100% as you can get without abstaining from sex altogether.
