Yes, but it is extremely rare.

Since a hysterectomy is reversible, there is always a chance, but it is extremely minor.

It's possible. Some doctors tie it the right way, but the woman still gets pregnant afterwards.

If you watch "I didn't Know I was Pregnant", a woman did get pregnant after getting her tubes tied. So in some cases it can happen. If your really sure you don't want any kids you should get your tubes cut, because in the future, your tubes can untangle.
