You can tell by looking around first of all. If the place just looks nasty, then it probably is. You should also research the place. Find people that have gotten piercings there before, and ask about what kind of experience they had there.
Also make sure the peron doing your piercing uses sterile needles, rings, etc. The needle should come out of a package. If it doesn't, then don't allow him or her to pierce you.
