I want to work in this clothing store.. but a few of the questions, I'm not sure how to answer.
Position: (Floor-sales person? ..what is the proper name? What are all of the positions in a clothes store? It's like a boutique.)
Date You Can Start: (How do I say whenever?)

Subjects of Special Study/Research Work, Special Training, and Special Skills?
What are some "Subjects of Special Study/Research Work, Special Training, and Special Skills" that a would be useful in a store?
Position: Sales
(Available positions would probably be: Sales, Stock, Cashier, Manager, Assistant Manager, Maintenance. Then there would also be: Daytime Sales, Evening Sales, Morning Sales, etc etc if you have a preferred time. Don't be afraid to list one or two.)
Date you can start: Immediately
(Immediately means, "I NEED this job." Otherwise you can say a certain date that is within a week away (then list: DAYOFWEEK 0#/##/2009)--but usually if you're looking for a job and you don't already have one then immediately is the case.)
Salary Desired: Negotiable.
(NEVER put minimum wage. NEVER. This means that will be all you get, even if they usually start you with $.50 more than that. This means that you KNOW you're not really qualified for anything. Negotiable means that you will see what they have to offer before you make a decision on how much that position is worth to you. If they're interested after you've put negotiable then expect them to start an offering of minimum wage but be VERY open to giving you more than that if you request!)
Special skills and what-not would be things that you have done that are, well, skills. Usually this can be simple things like the your WPM (words per minute) typing average, if you have office equipment training (ie: can operate a fax machine, photocopier, printer), if you can do spreadsheets. etc. If you have good communication and people skills then tell them (good for this job). If you have awesome organizational skills then let them know (good for this job). If you work well under pressure then let them know (good for this job). Don't lie though because you could be asked the dreaded question:
"What do you feel is your weakness?"
In your interview that can absolutely kill you if you've lied about your skills already.
After an interview, ALWAYS call them back within 2 days. That lets them know that you really are interested in the job.

Okay, For position you can put "Anything Available" that way they are aware that you are open to anything. If you want to be in sales, You can put simply put Cashier or Sales.
Date you can start, I always put A.S.A.P "As soon as possible"
Salary Desired, I always put "Minimum wage" That way they know you aren't too picky, and when they interview you if they ask what salary you desire you can possibly work something out with the management.

date you can start: as soon as possible might be a good way to say it.
Subjects of special study/reserch work, special training, and special skills would just be something that you have done or dealt with before that would make you good for this job like if your outgoing and good with people its a good thing to say for a position working with people.
