Anyway, if you get called back for an interview I suggest wearing nice tan, black, or brown slacks/dress pants.. whichever you'd like to call them. Also, shirt wise, nothing too "skanky" by any means. A nice shirt is all it really takes. You don't have to look amazing as if you were going for some sort of desk job, especially because you're only 15, but you do want to make a nice impression.
As for where to obtain a workers permit - where I'm from we get ours at school, or, if it's summertime, we can go to the DMV.

Your second question however:
If you are called back for an interview
- You always want to dress formal/semiformal -presentation is everything - make sure to be a little modest.
- Don't fidget with your fingers or anything while they are interviewing you
- Ask questions, it shows them that you are interested in them and the business
- If you don't have a resume already, have your mom or dad help you make one. Put on there things that would show that you have people skills, and you are able to deal with money. Even though this would be your true first job there is still alot someone your age can put on a resume. Babysitting, Things that you have organized for school, Volunteer jobs you've done. Anything that can relate to what you are applying for.
Handing your interviewers a resume before you start the interview gives them a great first impression, and already they will be impressed with you, before you have even opened your mouth (this applies to how you dress as well)
- If they ask you what flaws you have, make sure to point out something that can be turned into a good thing. such as... if you are a prefectionist. then that can be a bad thing if you focus on that too much, but when you learn how to control it, it can be really good. ect.
