Yes, tugging at the ear is a sign that it hurts. They don't have any other way to tell you they have an earache, other than crying, and I don't admit to know what my son is crying about sometimes, and I'm sure it is the same with any other mother out there.
Other than the fussiness and ear tugging, there really is no other way that you can tell what's going on inside his little ear. His doctor needs to examine his eardrums for redness. As I'm sure you well know, you won't be able to see that with your naked eye.
You should call his doctor ASAP and ask them what you should do. They will likely have you bring him in to the office right away to be worked in to see the doctor.
If he ends up with an ear infection, as my son has, he will be given a prescription for antibiotic to heal it up. (Most likely liquid you keep in the fridge.)
The antibiotic will make it feel better in a day or two, but be sure to give him the whole prescription to ensure the infection is gone.
