People who wear their "hearts on their sleeves" show their emotions freely and openly to everyone. They don't hold back their emotions, whatever the situation may be, good or bad. And the sleeve is sort of the area of the shirt where everyone can see it, so heart see it, sleeve see it, get it? I hope I helped.

It basically means that you openly express your emotions and aren't afraid of who sees it. Like if you literally had yor heart (where emotions come from) on your sleeve, everyone could see it.
Or, if you're like my friends, you make a heart skicker out of contact paper and put it on your arm :] That would be a literal translation.

lol...it means they are kind of very emotional and not afraid to show their true feelings...they wear their heart (which is like the place where your feelings come from) on their sleeve ( which means everyone can see it)., haha get it? it basically means they show their feelings
