Why do guys have to turn and cough when they get their physical?

Coughing temporarily increases internal body pressure. It helps the doctor tell if you have an inguinal hernia. You don't have to cough when the doctor is checking your prostate; that's a rectal exam, which is quite different.

Fortunately you shouldn't have to worry about that until you turn 40 or so. And to be honest, it's not really painful.
