It could be a number of things, among them:
Dislocated shoulder
Blood clot
Pinched nerve, etc.
Having freshly read about someone dying from a blood clot in the shoulder region, I would waste no time in getting to the doctor.
There have also been cases of people dislocating shoulders just rolling over at night.
