No, the condom wouldn't slip off the guy's penis because it was made to not slip and to stay on. But the condom can rip or break though.

Well, the condom is saposed to be rather tight on the male to prevent this from happening. If the condom is too loose, then I guess it could fall off. Eh, I dunno.
