When he gets hard, blood flows to his penis and his testicles, causing the penis to erect and the testicles to swell. If he doesn't ejaculate, then the pressure will build up in his testicle and cause it to swollen and kind of turn into a purple-ish color. (I don't know why it calls "blue balls" though; I might need to look into this afterward.)
This doesn't always happen though, depending on how "excited" he got.
Anyway, "blue balls" could cause the man to feel sore and mild ache, but if it is serious (like he got too excited and too much blood flowed in), then it could be as painful as being kicked "where-the-sun-doesn't-shine" (being kicked in the balls).
It could last anywhere from half an hour to over 5 hours.
But the good news is, it's not dangerous, and does no harm to him (except the pain). The blood will eventually drain on its own, if he doesn't ejaculate.

And yes, from a guy's point of view, it is mean. As much as you girls don't like your "monthly-annoyance" (your period [or PMS]), we don't like getting "blue balls" either.
