Masturbation is not considered sex. Sex is between two people. When an ob/gyn asks if you have had sex, don't say yes if all you have been doing is masturbating.
An ob/gyn won't knkw if you have had an orgasm before unless you tell them.

i just want to add to the answer below about the hymen- it can break very easily, so they wont be able to tell if you have had an orgasm, or masturbated, if your hymen is not attatched.

no, it's considered masturbating. and a gynecologist can not tell if you've ever had an orgasm, only if your hymen is stretched out (if your cherry is popped)
