No, it's not based off of Jon's life. The actual book takes place in the 50's or 60's (I can't remember). Nicolas Sparks did use some of his experience with his sister having cancer, but it's an original story.

No, it's not based on Jon Foreman. His band just sang the song in it. It's based on the book by Nicholas Sparks. Though they changed a few details to make it more of a teen romance movie.

In a speech he gave after her death in Berlin, the author admits, "In many ways, Jamie Sullivan was my younger sister." The plot was inspired by her life; Danielle met a man who wanted to marry her, "even when he knew she was sick, even when he knew that she might not make it". Both the book and movie are dedicated to Danielle Sparks Lewis."
