You have to change it because you can get TSS (Toxic Shock Syndrome). If you leave it in for too long, you're making it an easier taks for it to get a bacterial infection. And so you should follow what the box says, choose lowest absorbency for your flow, and alternate between pads and tampons. Don't sleep with it in at night either. And I believe it's 4-8 hours.
