Most people tend to not button the top button, unless wearing a tie. It looks very formal, and looks better in most cases when you don't.

Honestly, it doesn't really matter. Especially when you're wearing a tie, nobody notices (because the tie covers it). I'm a girl (so I don't typically wear ties) but most people like to be able to breathe and be comfortable. I never button the top button unless the shirt is loose without it.

So really, it's up to you. If your collar looks sloppy without it buttoned (and if that actually MATTERS, like if you're going to be interviewed for a job) then button it. You can always loosen it when you leave, right? But if you're dressed up for, say, a lacrosse game at school and have to wear a shirt & tie, nobody's gonna care that you didn't button your shirt, yaknow? Plus button-down shirts look nice (in my opinion) when the top's unbottoned.
