a. For many people it does hurt. Make sure you are relaxed and turned on and use some sort of lubricant.
b. Someone said this all ready but I'll say it again-only if your hymen isn''t fully torn( a tampon or injury can cause this).
c. Doctors can tell if your hymen has been broken, other than that it only stretches for a short period of time.

A. Depends on the person. Some people says it hurts, others do not think so. It depends on how relaxed you and your partner are. If it does hurt, it probably wouldn't be bad.
B. If your hymen has not been torn previously, yes it will bleed. Not much, though.
C. I'm not sure. If anyone, a OB/GYN could - but I'm not sure s/he would be able to. If they can, it's for a good reason - they went to school for 4+ years for it.
