It usually means making out or having sex with someone, as in 'Claire and Bob hooked up at that party.' But it can also mean that two people became and item/couple/whatever you prefer to call it. 'Claire and Bob hooked up a couple of months ago and they're still together now.' Some people use it instead of 'meeting up'. As in 'I'm hooking up with Claire and Bob later.'
