It has different meanings depending on the person. It usually means making out or having sex with someone, as in 'Claire and Bob hooked up at that party.' But it can also mean that two people became and item/couple/whatever you prefer to call it. 'Claire and Bob hooked up a couple of months ago and they're still together now.' Some people use it instead of 'meeting up'. As in 'I'm hooking up with Claire and Bob later.'

Some people think of the meaning different than the other as in:
becoming a couple
making out
having sex.

Hooking up is usually kissing or something, but not really being serious or dating. Pretty much just kissing without having any commitment.

"Hook Up" = Have sex with no relationship.
As in "No Strings."
It used to mean just getting together and hanging out with friends.
