Hello, my online name is Ralph SilverSky and of course that isn't my real name; my universal username for websites online is ThirdQED. Currently, I am a first-year student at a University in the USA, taking my first steps toward my dream of becoming a physician.

to make a form

that's all i got, i need it to sumbit that information entered in the text area. I need it to submit the information on the same page except in a table below the form. Also with that information in the text area, i need additional text merged with it to make an URL. That's all. Please give me accurate information, and information I can use. If you could give me html, that would be great. It would probably help me more. Thanks.

That looks like the setting of a contact/feedback form. Go to this webpage and scroll down: http://www.thesitewizard.com/wizards/feedbackform.shtml
It will guide you through the steps and give you all the stuffs you need.
First, it will provide you either a PHP or Perl script (depending on what you choose). Then you save the script to your computer and upload it to your website via FTP. It will then give you the html to put on your page.
The only thing that the HTML does is display the form. You can't make a form with HTML alone without a PHP or Perl script.
So if you have absolutely no idea what PHP or Perl script is, then I suggest that you either take time and learn about them first, or use a service where the script is not hosted on your site.

EmailMeForm doesn't require you to upload any script to your site, all you need to do is embed the HTML and that's it. Pretty easy for a beginner.
