Given the fact that you are in serious pain you need to see a doctor. This could be anything from a clogged oil duct needing to be lanced and drained, to something far more serious. We can't say as we are not doctors. Even if we were you can't make any type of diagnoses over the web.
Given the fact that you are in serious pain see if your family doctor will give you an emergency same day appointment. If not there are 24 hour walk in clinics staffed by doctors. Call one of them and see if you can make an appointment with them. While they are called walk-in clinics many will see you on a appointment basis that way you are not waiting around to see the doctor. Of course there is always the hospital emergency room that you can go to if needed.

Wash it frequently DO NOT SQUEEZE OR POP IT it will only make the problem worse. It should go away within a week or two with this, if not try calling a dermatologist and go in for an appointment. It won't need surgery but perhaps a little medication. You can also use honey to soothe the pain, or benadril cream, or the benadril pill to help the swelling. It is not a big problem and should go away quickly. [ brenalair's advice column | Ask brenalair A Question ]

However, you did mention serious pain rather than mild to moderate discomfort and that's of concern.
I don't know how soon you can see a doctor in the U.S. It's a different system in Canada where it's free and you can see whomever you wish or go directly to a clinic and be seen immediately.
I know where you are that's not an option and insurance companies dictate what you can and cannot do. What I would do if this was scaring you try to figure out how long it was, your symptoms, the pain and then go to an ER. Explain it to them and that you're concerned and obsessed with needing an answer. They will definitely have an ear, nose, throat expert which is what you need to be seen by ultimately.
