I will not attend on class for two days and i want to ask permission. How could i write permission note?

It depends on the teacher. If you see her then talk to her. Tell her the reason you will be missing class the two days and if there is anything you can do to make up the work and not lose points on anything. If it's a medical reason, tell her you will bring the doctors note. If it's a family emergency or the like, ask your parents or guardians to write a note explaining the reason.
