How do you fill out a w-9 form for youtube partnership?

I need to submit my w9 form info, to that they can release my acount to be get paid... I don't understand how to fill it out. Does that mean I have to pay taxes at the end of the year from the money I make from youtube?

if I submit one do I have to pay taxes myself or will the taxes be deducted once I receive my money on paypal?

If I click on the box that says I give 28% of my earnings to the IRS does that mean I still have to file for and pay for taxes? I'm confused.
