Perhaps it is best to start at the beginning and ask if it is something he really wants and when?. I have come to learn that everything has its time and planning a family can put a couple at odds about the best time to have a baby.
In my relationship we both want a baby (or two) but at the same time we have sat down and said that we need to start saving and get ourselves prepared and settled before we bring someone else into our lives as having a baby is about both of you.

I don't know. It's not something people say. Like, if he turned around and said "I hate you", then we can easily tell you that he probably just got angry, and doesn't actually hate you.

"Why do you think that you haven't gotten pregnant" is not something people just say. I've never heard it. The best thing is to ask him what he means buy it. Just ask in passing what he means, as you genuinely don't know.

Good luck!
