what could cause a man to carry a grudge towards a female acquaintance for 4 years, when he did not even explain to her what she did to hurt him?
Why does he seem to sabotage her and outcast her on top of it? Why does it take him so long to move pas things and just let her know what she did?

Well only the person knows about that really. The best option is to clearly speak to him in a calm and reasonable way, however uncomfortable it may make you feel.
So do speak to him, and see what he has to say. Try and resolve the issue at hand, because only he really knows what the problem is (if any).
