It seems that you don't need to pay any special monthly fee for the 3G since it comes with the Kindle for free. You can use it to download books or even surf the internet. The extra $50 that is charged with the Kindle 3G, compared to the Kindle WiFi, covers that basic 3G cost.
Straight from the Amazon page it does say:
"No Monthly Wireless Bills or Annual Contracts
No monthly wireless bills or commitments. Amazon pays for Kindle's wireless connectivity so you won't see a monthly wireless bill. There is no wireless setup – you are ready to shop, purchase, and read right out of the box." from:
