very possible, haha. it could be too tight to fit a thicker penis. or maybe it doesn't go deep enough for a longer penis to fit it, it's usually the tightness though.

In terms of length, yes. The average vagina is 5-6 inches deep (Could be slighly deeper depending on the angle of entry). After that, the penis is hitting cervex, which is unpleasent for all parties involved.
In terms of width, maybe initially, but vaginas are pretty amazing in their ability to stretch and accomodate. They are made to allow babies to pass through.
