My mother is a physical therapist, and I have been helping take care of my Great Grandmother for several years now while she has been in and out of the hospital. I tell you this so you have some reassurance that I at least somewhat know what I am talking about.

First off, like the above users said, if she falls ask her if anything hurts, and make sure she hasn't seriously injured herself.

If she has call 911.

If she tells you she is more or less ok, and it is just a matter of getting her off the floor, you need to know how to do that without hurting her.

Do not grab her hands and pull her up like you would one of your friends.

If you think you can lift her yourself, then do these next instructions alone, if not call your cousin and get her to help you.

Back the the don't pull her by her hands. go behind her and lift her up into a sitting position, in such a way that if she had the strength she could stand up.

Then you grab under her arms, in the armpit, and that is where you do the lifting, this will prevent and dislocating of joints etc. (You would do this kind of the same way you pick of a toddler.)

Honestly in order to get her up you are most likely going to need someone else there just in case because you don't want to get her halfway up and then not be able to hold her, because she would end up falling again.
Hope this was helpful.

if she falls down, BE NEAR HER. ask her if she hurts anywhere, and if she says no, then -- and only then -- should you help her up yourself.

if she tells you she is hurt, call your cousin and let him / her decide whether to go to the emergency room.

but you should always try to avoid having her fall down in the first place. buy her an emergency button so if she falls down and you're not there, she can press it and the ambulance can come immediately.

always try to help her do her everyday tasks and be near her in case she needs you.

If Grandma falls down, I would call 911. You never know if she might have broken a bone or not. Moving her yourself can possibly cause more damage to something that could be broken.
