most liekly not with just a signature. normally they have to take you in and sign the paper THERE. otherwise you could just easily forge it!
also, some parlors dont even do it to minors... but it all depends on the tattoo parlor. so call and do research about where your gonna get it first, and normally the tattoo parlors also have websites, in which they give contact info (either a phone # and/or email)

with a parent's signature, definitely yes. in some places, i believe you can get one when you turn 16 without a parent signature though. it depends on where you go. a good idea would be to call a tattoo parlor and ask them to make sure before you go.
