A true birth certificate contains what ygsgirl said.
though I don't know how much of that info would have been on one in those times.
My Grannys moms birth certificate was from Czechoslovakia and was black paper with white writing. So in those days you would probably want to research what the used to write with and what they would have written on. :) [ karenR's advice column | Ask karenR A Question ]

Date of birth
Time of birth
Gender
Baby's full name first, middle, last
Place of birth, such as hospital, home, barn, etc
City/town of birth
Mother's full maiden name
Mother's age at time of baby's birth
Father's full name
Father's age
Mother's residence- address, city, state, etc
Mother's country of birth
Father's country of birth
Mother or father's signature and date of signature

certificate
1. you can put what day he was born on.
2.his mothers name & fathers name
Also put his name on it !
