Bras are to keep breasts from moving around too much. When I don't wear a bra, it hurts, and the skin underneath the breasts is chafed. Likewise, wearing jeans without underwear would scrape the skin of my vulva. It also keeps pants clean. It is also considered impolite and taboo in my society to not wear these things.
Nipples do get hard when they are excited. But they're not very sharp.
