16, 5'4", 135-140 lbs (it fluctuates).

If I eat healthy (cutting out junk foods such as candy, chips, soda and all that other stuff), do cardio (walking, to slow jog, to running for a few minutes, and switching around all of those, since I can't run because I have low stamina) for atleast 30-50 minutes, 3 times a week.

How much weight could I realistically loose in 3 weeks or a month, I do lose weight relatively fast, but gain it back... because I'm lazy haha.

probably 6-8 in a month.

If you're truly serious about it, you should lose about 2-3 lbs a week. As long as you continue to be healthy, you'll continue to lose weight -- but you don't need to lose a lot. The healthy weight for your height is about 108-145 lbs, with the ideal being at about 120-130.
