There's a very, very, very slim risk of a woman transmitting HIV during a blow job. First, she has to have blood in her mouth and, second, he has to be actively bleeding from somewhere on his penis. Then the blood has to mix and her blood has to get up into his blood stream. It's highly unlikely.
