When I was working at a doctors office the patient would get a form from the place where you get your car tags (I'm wanting to call it the motor vehicle dept. but don't think that's right)in the county courthouse.
Then the form has to be taken to your doctor and he/she will fill it out and sign it for you.
BrokenWings answered Tuesday November 22 2005, 6:16 pm: It isnt actually being classed as disabled which intitles you to one, its whether or not you can walk 100 meters unaided or without considerable amounts of pain.
