I have this friend who thinks shes fat what should i do?

The best you can do is try to boost her self esteem a bit.

Tell her she's pretty, tell her she's awesome, and if the problem progresses, tell an adult.

Well when girls say there fat most times it because of low self esteem so basically convince her in anyway that shes not tell her shes pretty and that she needs to stop putting herself down like that because its not healthy and could lead to depression and if that doesnt work then ask her what would help her make feel better about herself and if she truly feels like she is fat then just tell her that if it makes her feel better you both can go to the gym and start working out and tell her that even you want to lose a few pounds (this doesnt have to be true)basically do whatever you can to make her feel good about herself.
