Neither visual nor manual inspection of the genitals will tell you - although if a woman has been inserting progressively larger items into her vagina without doing Kegal exercises it may stretch out a bit (which is only noticeable if you examined it beforehand, as all women start out different sizes and tensions, and if the change is pretty drastic because - like the rest of the body - the vagina does change with time).
