I can think of only two ways for someone to be sure that you masturbate.
1. they walk in on you and see you masturbating.
2. you tell them and they believe you
3. you've been making a lot of noise and they heard you

People who know you really really well and who are paying close attention to such things may be able to notice that your scent changes slightly when your turned on - but that doesn't mean you've done anything to cause it or relieve it. If you don't clean it off, semen and the natural lubricants produced in the vagina have a distinctive scent, but cleaning up after yourself by wiping off with toilet paper is enough to take care of that. If you use a scented lubricant - and they know what that lube smells like - it's possible for them to pick up the scent.

Neither visual nor manual inspection of the genitals will tell you - although if a woman has been inserting progressively larger items into her vagina without doing Kegal exercises it may stretch out a bit (which is only noticeable if you examined it beforehand, as all women start out different sizes and tensions, and if the change is pretty drastic because - like the rest of the body - the vagina does change with time).

So yeah, if your worried about someone finding out, they've pretty much gotta walk in or listen in.
