There are obvious answers I've heard that apply to many others who use the same statement. Although, in the end, to really understand him best, it might be best to ask him for examples of what it means to him. It may mean some but not all of the following:

I am a touchy-feely personality type.
I get very emotional really easily.
I openly display my feelings.
I am an open book, I don't hide any secrets.
I am not afraid as a male to shed tears.
I can be hurt very easily.
My emotional state is written all over my face.

It means 'when a person openly displays his/her feelings instead of hiding them'

Nothing hidden. No secrets.
