At the end of the first season, Tony gets hit by a bus. As a result he is suffering from subdural hematoma, which is a serious blood clot on the brain, leading to his learning difficulties, amnesia and personality changes. In season one, Tony was a very popular and well known guy but is now a virtual recluse and is severely depressed.
I can't remember that exact part of the episode but I think what 'they know' is about his accident and subsequent brain damage.
