My brother is diabetic and he accidentally hit his head while getting in the car did not paid too much attention to it until a couple days after when we call 911 for an ambulance because he was really bad. It seems that he was bleeding internally due to the hit in the head and did not know it, that was the explanation the Doctor gave us, he undergo an operation and after 24 hours he seem completely well until four days after the operation when he could not speak any more, the doctor requested to a nurse to give my brother some medication for seizure and he responded well, but after a few hours he was back to the same condition. Now they are checking that all the nerves in his head are working normal and after 20 hours they will let us know what it is the matter with him. Is any one out there that may have some advise for our family?

It is not unusual for someone with a closed head injury not to even realize they did damage right away. If your family is feeling guilty because they did not notice something sooner, don't be. It would be a very rare thing for someone to get such an injury just from hitting their head getting into the car. There is no way you could have known.
One of the complications from any surgery is blood clots. I am not saying your brother has or had one because I am not a doctor. Blood clots especially in the head can lead to strokes. There are meds that can be given to dissolve and break up a blood clot and blood thinners might also be used.
The main thing is to question his doctor and/or his nurse. Whichever is available to you at the time. The doctors do want you to understand what is going on.
