It can be caused by a number of things but the angle at which he enters you is probably the most likely culprit. When you're on top you control everything, so you decide angle, roughness, etc.
I think you guys should try some different positions. I know missionary can NEVER get me there and I'm too lazy to be on top long enough to get me there. My man an I favor doggie style and sometimes the spider (which I honestly think is too much work, ha).
You just have to find a position that works for you. Every body is different and needs different stimulation.
