I pack all of my school supplies up in my new backpack and pick out my outfit for the first day of school. Cliche, I know, but that's what I do =P
I also try to start waking up earlier about 2 weeks before school starts. Every morning, I wake up a little earlier. I started at 7:45, then went to 7:30, then 7:15, etc etc.
My morning routine is usually;
1. Wake up and open my blinds/make my bed.
2. Wash my face.
3. Get dressed and do my hair.
4. Eat breakfast/take my vitamin.
5. Brush my teeth and get my stuff ready.
6. Make my lunch for school.
7. Leave for school.

1.) i always get EVERYTHING done asap! like as soon as i possibly can. i work well when i have a nice long list in front of me and i check each thing off as i do it. try making a list and setting short deadlines for yourself.
2.) wake up at 6, wash face / brush teeth, make-up, get dressed, eat breakfast some days, leave for school :) ]
To prepare, really the only thing I do is to start waking up earlier every day, but in the past few years, I haven't been doing that, and I've been able to get up just fine. So try waking up a half hour earlier each day until you get to the time you'll have to wake up, that is, unless you didn't sleep in over summer.
My morning routine is usually this:
1. Hear my alarm go off.
2. Hit snooze button.
3. Wake up at 6:00.
4. Jump in the shower.
5. Shave.
6. Get dressed.
7. Eat breakfast.
8. Make my lunch.
9. Leave for school.
