2) And when in surgery must nipple piercings be taken out?
3) And will any injections have to taken? And does an IV hurt?
4) -only if you had a boob job done- what's an estimate (1-10) on how much it hurts over all?

Anyways, you would go through a consultation with the doctor to "try" the boobs on and see what's best for you. However, you should listen to the doctor because he/she's the expert here. There's such a thing as going too big, in that case your boobs will completely go to opposite sides and look bad when not in a bra.
I'm pretty sure you have to take the piercings out.
I'm 100% sure you have injections and IV. You undergo anesthetics and are given painkillers, before and after surgery. You probably won't even feel the IV because you'll be asleep, but anyways, IV's don't hurt, they just feel SUPER uncomfortable.
I've never had a boobjob before, so I can't speak from personal experience about the pain, but you won't feel anything during surgery. Afterwards, it's a lot of swelling for the first few weeks, but after that you're good.
