how to do this in picnik

I want to make a picture like this.
I know how to do it on one separate part, but how do you do it in different spots as well?
Please only answer if you are familiar with Picnik.

If you don't know, then please don't answer..
I have seen many people do it in Picnik, and I want to know how.
You can use Photoshop, AND Picnik. I'm asking how to use PICNIK, not PHOTOSHOP.
