i found a site that has the cause and info you are looking for

but from what I know it's very rare.. but it's more dangerous when using high absorbant tampons and keeping them in longer then the approporiate time it says on the box.
the symptoms I found from that website are :
high fever (over 102° F.), vomiting, diarrhea, a sunburn-like rash, and drop in blood pressure. Muscular aches and pains, headaches, sore throat, blood shot eyes, confusion, and peeling of the skin on the palms and soles may also appear

Its caused when you leave a tampon in for too long; longer than eight hours, I think.
Symptoms:
-High fever; about 102 degrees.
-Rash.
-Headache.
-Muscle aches.
-Sore throat.
-Cough.
-Nausea and vomiting.
-Diarreha.
-Abdominal pain.
-Lightheadedness or fainting.
-Confusion or disorientation.
-Low blood pressure.
If you leave a one in for too long and you get any of these symptoms, go to a doctor immediately. It can be life-threatening.
