I had sex. I told my doctor b.c. she said it was confidential. She told me she was going to do a pap-smear next time we met. I'm totally against it because I'm not having sex again untill I'm married. Can I like .. refuse to have one?

Of course! You have the right to refuse treatment of any kind at any time.
And, yes, what you tell her is between you and her under the doctor/patient confidentiality rule.

Though it isn't a bad idea to let her check you out to make sure everything is in proper working order, even though you don't plan to have sex again until marriage.

But you don't have to. It is your choice, not your doctor's, or anyone else.
