They will ask all kinds of questions
and ask if you have any questions.
If you do, then is the time to ask.
They won't tell mom you asked, what you
talk about is between you the doctor and
the nurse.
Here is a website that will explain the
whole exam and visit for you. If you think
you might be nervous (and its normal so you
will be!) write down any questions you may
have so you don't forget. :)

They ask you if you have had sex and if you used protection.
And if you use a tampon. If you are currently on your period.
Just questions like that.
No need to feel embarrassed, It's their job.
And if there is something you don't want your parents to know. Then you can tell the doctor that there is no need to inform your parents. And they have to honor that, or they can get fired.
